Cardiac Electrophysiology Essentials

Review the conduction system, sinus-node activity, atrioventricular transmission, bundle branches, ventricular activation and repolarisation. Understand how disorders of impulse formation or conduction can cause bradycardia, tachycardia, heart block, atrial fibrillation and other arrhythmias.

Connect rhythm findings with symptoms including dizziness, syncope, palpitations, fatigue, chest discomfort and breathlessness. Learn why an ECG or device recording should be interpreted alongside the patient’s observations and clinical condition.

Pacemakers & Pacing Therapy

Explore the components of a pacemaker system, including generator, leads, sensing and capture. Learn the broad indications for pacing in sinus-node dysfunction, symptomatic bradycardia and selected conduction disorders.

  • Single- and dual-chamber pacing concepts.
  • Sensing and capture thresholds.

Programming requires specialist training. This course develops understanding and communication skills; it does not authorise independent device adjustment.

Understand how ICDs identify and treat potentially dangerous ventricular arrhythmias. Explore pacing, cardioversion and defibrillation, as well as the broad concepts of detection zones, stored episodes and inappropriate shocks.

Learn how to respond when a patient reports a shock, collapse, palpitations or repeated therapy. Patient education should include appropriate follow-up, emotional support, activity advice and referral to the specialist device team according to local guidance.

Cardiac Resynchronisation Therapy

Explore how CRT can improve the timing of ventricular contraction in carefully selected patients with heart failure and electrical dyssynchrony. Review CRT-P and CRT-D, lead-placement concepts, response assessment, ECG criteria and the importance of individualised selection.

CRT response is not guaranteed. Patients require ongoing clinical review, imaging where appropriate, medication optimisation and specialist follow-up.

Device Review & Interrogation Awareness

Build awareness of information considered during a device review: battery status, programmed settings, sensing, capture, lead impedance, stored arrhythmia episodes, pacing burden and delivered therapies.

  • Confirm device type and clinical indication.
  • Review trends and compare with previous checks.
  • Correlate device data with symptoms and ECGs.
  • Consider medication, electrolytes and clinical context.
  • Escalate unexpected or concerning findings.

Remote monitoring can identify alerts early, but alerts must be reviewed and acted upon by an appropriately trained clinical team.

Holter Monitoring & Loop Recorders

Understand the role of ambulatory monitoring in intermittent palpitations, unexplained syncope and suspected rhythm disorders. Compare the purpose and limitations of Holter monitors, event recorders and implantable loop recorders.

The relationship between symptoms and recorded rhythm is important. Encourage patients to record timing, activity and symptoms accurately, then ensure results are reviewed through the appropriate pathway.

Complications & Safety

Recognise broad complications such as infection, haematoma, lead displacement, perforation, venous obstruction, battery depletion, sensing problems, inappropriate therapy and device malfunction.

Urgent review may be required for collapse, severe chest pain, new breathlessness, repeated shocks, fever at an implant site, unexplained syncope or signs of poor perfusion. Follow local emergency procedures and seek specialist support without delay.

Peri-Procedural & Multidisciplinary Care

Cardiac-device patients may be seen by cardiology, electrophysiology, emergency medicine, anaesthesia, surgery, nursing, imaging and rehabilitation teams. Safe handover should include device type, indication, symptoms, recent events, anticoagulation status, relevant investigations and the plan.

Before procedures or imaging, follow local device and electromagnetic-interference guidance. Never assume that a device is safe for a procedure without appropriate confirmation.

Patient-Centred Communication

Patients may worry about implantation, shocks, travel, work, exercise and future health. Practise explaining technical information in plain language, checking understanding and respecting consent, privacy, culture and individual priorities.
